We are seeking a skilled Registered Nurse to join our team as a Case Manager at Trinity Hospice of Aiken. As a Case Manager, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
Key Responsibilities- Initial Nursing Evaluation: Conduct thorough assessments of patients to determine eligibility for hospice services within 48 hours of referral.
- Plan of Care Development: Identify patients' physical, psychosocial, and environmental needs, and re-assess as needed, no less than every 15 days.
- Collaboration and Communication: Work closely with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG) to provide comprehensive care.
- Documentation and Record-Keeping: Maintain accurate and relevant clinical notes regarding patients' conditions.
- Experience: One year of clinical experience as a Registered Nurse.
- Licensure: Current RN licensure in the state of practice and one year of clinical experience.
- Certifications: Current CPR Certification.
- Transportation: Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
