Overview:

Position Summary: In this position, you will play a crucial role in delivering compassionate care to patients facing life-limiting or terminal conditions. Collaborating closely with an interdisciplinary team, you will provide guidance and support to the nursing staff, ensuring that patients receive the highest quality of care during their most challenging times. This is a Hospice Role

Key Responsibilities:

  • Monitoring essential patient metrics, including temperature, heart rate, respiratory rate, and blood pressure.
  • Administering prescribed medications and treatments as directed by healthcare providers.
  • Alerting physicians to any significant changes in patient conditions.
  • Providing emotional support and counseling to family members, preparing them for the progression of their loved ones' conditions.
  • Informing families about available resources, including counseling services and government assistance programs.
  • Assisting families with necessary documentation and paperwork.
  • Maintaining accurate and up-to-date patient records.
  • Adhering to established infection control protocols, standards of care, and patient confidentiality regulations.
  • Complying with the organization's personnel policies and procedures.
  • Performing additional related duties as required.
  • Utilizing keen observation skills to detect changes in patients who may be unable to communicate effectively.
  • Exhibiting strong verbal communication skills to coordinate with other healthcare team members and report to physicians.
  • Demonstrating well-developed interpersonal skills to foster positive interactions with patients and their families.
  • Possessing knowledge of end-of-life care standards.
  • Being familiar with pain management medications and treatments commonly utilized in hospice care.
  • Proficient in computer skills, including email communication and patient record management software.
  • Capable of assisting patients and handling medical equipment, such as IVs and nebulizers.

Qualifications:

  • Experience in hospice care is required.
  • Current LPN License in Pennsylvania.
  • Valid CPR certification.
