Position Overview:


The Residential Program Coordinator at Lodging Plus Services plays a pivotal role in working alongside team members to ensure the effective daily management of the residential aspect of the Lodging Plus initiative.

In collaboration with the care team, the Coordinator oversees the therapeutic environment for patients, employing analytical skills and established best practices.

This role requires a comprehensive understanding of clinical resources and the ability to address patient behaviors and care requirements effectively.

Key Responsibilities:

Operational Duties:
Facilitates a seamless admission process for patients entering the Lodging Plus program.

  • Introduces new patients to the Lodging Plus program.
  • Monitors adherence to program admission standards.
Oversees the daily functions of the Lodging Plus program.

  • Provides direction and support to team members.
  • Conducts safety assessments of patients through visual checks and documentation.
  • Manages room and personal belonging inspections.
  • Engages in de-escalation strategies for patients, utilizing knowledge of specific diagnoses and best practices.
  • Actively participates in managing the therapeutic environment by addressing early signs of behavioral escalation.
  • Ensures patient supervision.
  • Consistently applies program rules throughout each shift.
  • Timely escalates concerns regarding patient behaviors.
  • Demonstrates understanding of signs and symptoms of various diagnoses and appropriate interventions.
  • Leads community meetings for Lodging Plus patients.
  • Organizes and facilitates recreational activities for patients.
  • Conducts educational sessions with individuals or groups as per treatment plans, documenting participation accurately.
Assists clients in accessing medical services.

  • Ensures the security of medications within the Lodging Plus program.
  • Facilitates patient access to necessary medications in accordance with program policies.
  • Aids patients in making informed decisions regarding emergency care.
Identifies issues and implements effective solutions.

  • Responds to emergencies in accordance with established training.
  • Addresses concerns raised by patients and their families.
  • Communicates specific patient issues to relevant staff, including counseling, nursing, program administration, and security as needed.
Promotes operational efficiency and effectiveness.

  • Meets expected productivity and efficiency benchmarks.
  • Demonstrates the ability to prioritize tasks effectively.
  • Utilizes resources, including supplies and equipment, in an efficient manner.
Advocates for Lodging Plus services.

  • Collaborates with other programs within Fairview Behavioral Services.
  • Represents Lodging Plus positively to partner hospitals.
Encourages a culture of evidence-based practice through participation in various activities:

  • Implements evidence-based policies in clinical settings.
  • Engages in learning opportunities focused on evidence-based practices.
  • Acknowledges patient involvement in academic research initiatives.
Demonstrates the ability to tailor care approaches to meet the developmental and cultural needs of the served population.

  • Collaborates with patient caregivers in decision-making processes.
  • Communicates respectfully with all individuals.
  • Ensures a safe and secure environment for patients.
  • Customizes care plans to address individual patient needs.
  • Adjusts clinical interventions based on the population served.
  • Provides educational resources tailored to the learning needs of patients and caregivers.
Exhibits the capability to enhance evidence-based practices by applying standards, guidelines, and care pathways. Integrates data and insights into practice to continuously improve care delivery and patient outcomes.

Qualifications:
Required

Education

  • Bachelor's degree in a relevant human services field.
Experience

  • Minimum of one year of experience in a human services role.
Preferred

Experience

  • One year of experience in services related to substance use disorders.
