Nurse Practitioner

Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our team at HealthCare Talent. As a Level of Care (LOC) RN, you will conduct home visits to assess the needs of prospective enrollees and provide direct and indirect nursing care to assigned PACE participants.
Responsibilities:
- Attend Interdisciplinary Team meetings to communicate prospective enrollee cases for approval.
- Conduct initial home visits to carry out nursing evaluation for Level of Care assessment.
- Complete initial and annual Level of Care assessments and send to the State for approval.
- Record participant assessments, reassessments, and care delivered in the medical record.
- Review all necessary documentation and coordinate with other disciplines as needed for bi-annual LOC audit preparation.
- Remain attentive to participants' health status and social functioning.
- Assist with identification of overt problems/needs and set priorities.
- Provide direct care as assigned by the Clinic Supervisor RN, including wound care, IV therapy, blood sugar checks, and vital sign checks.
- Consult with and educate participants, family members, and caregivers regarding disease processes, self-care techniques, and medical interventions.
- Provide the Primary Care Physician with a summary of health findings and any changes in a participant's health condition.
- Ensure that all tests, lab work, and diagnostic studies ordered by the Primary Care Physician have been carried out.
- Coordinate all medication for BCSC, including stocking non-prescription medications and arranging for delivery of needed medications and supplies to the participant's home.
Requirements:
- Fluent bilingual Spanish/English skills are required.
- Graduate of a school of professional nursing (ASN, BSN, MSN).
- Current License as RN through California Board of Registered Nursing in good standing.
- Verification of completion of CPR (BLS) and first aid training.
- Valid California driver's license and reliable vehicle.
- At least one year of experience as an RN, preferably in home health or hospice working with the frail or elderly.
- Comfortable and confident in providing in-home assessments and care for geriatric patients and making phone calls.
- Excellent computer skills (MS Office) and strong communication skills.
