Job Title: Registered Nurse

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our team at HealthCare Talent. As a Level of Care (LOC) RN, you will conduct home visits to assess the needs of prospective enrollees and provide direct and indirect nursing care to assigned PACE participants.

Responsibilities:

  1. Attend Interdisciplinary Team meetings to communicate prospective enrollee cases for approval.
  2. Conduct initial home visits to carry out nursing evaluation for Level of Care assessment.
  3. Complete initial and annual Level of Care assessments and send to the State for approval.
  4. Record participant assessments, reassessments, and care delivered in the medical record.
  5. Review all necessary documentation and coordinate with other disciplines as needed for bi-annual LOC audit preparation.
  6. Remain attentive to participants' health status and social functioning.
  7. Assist with identification of overt problems/needs and set priorities.
  8. Provide direct care as assigned by the Clinic Supervisor RN, including wound care, IV therapy, blood sugar checks, and vital sign checks.
  9. Consult with and educate participants, family members, and caregivers regarding disease processes, self-care techniques, and medical interventions.
  10. Provide the Primary Care Physician with a summary of health findings and any changes in a participant's health condition.
  11. Ensure that all tests, lab work, and diagnostic studies ordered by the Primary Care Physician have been carried out.
  12. Coordinate all medication for BCSC, including stocking non-prescription medications and arranging for delivery of needed medications and supplies to the participant's home.

Requirements:

  • Fluent bilingual Spanish/English skills are required.
  • Graduate of a school of professional nursing (ASN, BSN, MSN).
  • Current License as RN through California Board of Registered Nursing in good standing.
  • Verification of completion of CPR (BLS) and first aid training.
  • Valid California driver's license and reliable vehicle.
  • At least one year of experience as an RN, preferably in home health or hospice working with the frail or elderly.
  • Comfortable and confident in providing in-home assessments and care for geriatric patients and making phone calls.
  • Excellent computer skills (MS Office) and strong communication skills.
