Job Summary

As a Licensed Practical Nurse (LPN) at Apreva Hospice, you will play a vital role in providing direct patient care and coordinating with our interdisciplinary team to ensure our patients receive the highest level of care. With a focus on pain and symptom management, you will work closely with patients and their families to create a personalized care plan that meets their unique needs.

Responsibilities
  • Provide direct patient care, including pain and symptom management, to hospice patients.
  • Implement nursing practice following a comprehensive assessment and plan of care.
  • Document patient services accurately and in a timely manner.
  • Assess and educate patients and their families/caregivers on their care plan.
  • Participate in the coordination of hospice services.
  • Manage pain and symptoms effectively and evaluate patient response to treatments/medications.
  • Participate in professional growth and development opportunities and quality assessment activities.
  • Ensure compliance with all state and federal legal and regulatory requirements.

Qualifications
  • Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation.
  • 1 year of nursing experience required, 3+ years preferred.
  • Experience in community health, hospice, or med/surg preferred.
  • Knowledge of the hospice philosophy of care.
  • Commitment to clinical and documentation excellence.
