As an RN Case Manager, you will work closely with physicians, social workers, and other healthcare professionals to develop individualized care plans and provide direct patient care. Your strong clinical background, excellent communication skills, and ability to manage multiple cases simultaneously will be essential in supporting patients and their families through difficult transitions.
Key Responsibilities:
- Assess and manage the clinical needs of hospice patients, including initial and ongoing assessments.
- Develop, implement, and adjust individualized care plans for hospice patients.
- Provide direct patient care, including pain and symptom management, wound care, and medication administration.
- Coordinate care with interdisciplinary team members, including social workers, chaplains, and therapists.
- Educate patients and families about disease progression, treatment options, and care goals.
- Document all patient care activities accurately and in a timely manner.
- Participate in case conferences, care planning meetings, and other collaborative activities.
- Ensure compliance with hospice care regulations and company policies.
- Act as an advocate for patients' wishes and needs, supporting them and their families during end-of-life care.
Requirements:
- Current Registered Nurse (RN) license in CA.
- Minimum 1 year of experience in hospice, palliative care, or related healthcare field.
- Strong understanding of hospice philosophy and palliative care.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a caseload of patients.
- Knowledge of Medicare/Medicaid and hospice care regulations.
- Compassionate, patient-centered approach to care.
- Valid driver's license and reliable transportation.
