Job Summary

The Home Health Intake Supervisor is a key member of the Compassus team, responsible for overseeing the referral management process and ensuring the highest level of quality care for our patients. This role requires strong leadership and communication skills, as well as the ability to analyze data and make informed decisions.

Key Responsibilities
  • Leadership and Team Management: Lead, mentor, and motivate a team of Home Health Admission Coordinators to ensure they meet performance standards and goals.
  • Referral Management: Oversee the referral intake process, ensuring timely and accurate processing of patient referrals.
  • Quality Assurance: Perform regular patient chart audits, generate reports, and analyze referral data to identify trends, areas for improvement, and opportunities for efficiency.
  • Process Improvement: Make recommendations for process enhancements based on data analysis.
  • Communication and Collaboration: Collaborate with agency leadership, case managers, and growth leaders to obtain necessary information for referrals.
  • Regulatory Compliance: Stay up-to-date with regulatory guidelines and ensure all referrals adhere to legal and compliance standards.
  • Employee Development: Coach employees, provide input for performance evaluations, counsel and provide disciplinary actions to assigned personnel in conjunction with the manager, and work to facilitate individual and team development that drives positive results.
Requirements
  • Education: Associate or Bachelor's degree in Nursing required.
  • Experience: Minimum of two (2) years of experience as a registered nurse and one (1) year in home care required.
  • Skills: Strong leadership and communication skills, ability to analyze data and make informed decisions, and ability to work in a fast-paced environment.
Working Conditions

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
