CenterWell is seeking a skilled and compassionate Clinical Care Coordinator to join our team. As a key member of our home health care team, you will play a vital role in ensuring the highest quality of care for our patients.
Key Responsibilities:
- Conduct thorough assessments of patient needs and develop personalized care plans to meet their unique requirements.
- Collaborate with healthcare professionals, including physicians and therapists, to ensure seamless care coordination and optimal patient outcomes.
- Coordinate and manage the care team, including scheduling and disseminating updated clinical information.
- Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure the best possible outcomes.
- Stay up-to-date on the latest clinical policies, procedures, and regulations, including PDGM, to ensure compliance and quality care.
Requirements:
- Bachelor of Science degree in Nursing (BSN) or equivalent experience.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification and excellent verbal and written communication skills.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Ability to read, write, and speak fluent English.
