Key Responsibilities:

  • Conduct thorough assessments of patient needs and develop personalized care plans to meet their unique requirements.
  • Collaborate with healthcare professionals, including physicians and therapists, to ensure seamless care coordination and optimal patient outcomes.
  • Coordinate and manage the care team, including scheduling and disseminating updated clinical information.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure the best possible outcomes.
  • Stay up-to-date on the latest clinical policies, procedures, and regulations, including PDGM, to ensure compliance and quality care.

Requirements:

  • Bachelor of Science degree in Nursing (BSN) or equivalent experience.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Current CPR certification and excellent verbal and written communication skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Ability to read, write, and speak fluent English.
