About the Role

Givers is seeking a highly skilled and compassionate Health Coach to join our team. As a key member of our organization, you will play a vital role in providing training, guidance, and support to family caregivers.

Key Responsibilities
  • Client Eligibility and Resource Assistance: Assist clients and primary family caregivers in determining eligibility for assistance and resources within the Medicaid Waiver program.
  • Family-Centered Assessments: Conduct family-centered assessments and identify support needs for caregivers to help provide care at home.
  • Ongoing Health Coaching: Provide ongoing health coaching training and support to family caregivers to ensure client goals are met.
  • Communication and Collaboration: Maintain effective communication with primary family caregivers, registered nurses (RNs), and Medicaid waiver case managers.
  • Progress Monitoring: Monitor family caregivers' daily progress notes and tasks performed via the Givers mobile application.
  • Supervisory Calls and Case Conferences: Conduct monthly supervisory calls and semiannual in-person case conferences with clients, caregivers, RNs, and care coordinators.
  • Client File Management: Prepare and manage client files with a focus on accuracy and compliance with regulatory standards.
  • Community Partnerships: Collaborate with the Agency Operations team to ensure caregiver files are compliant with regulatory standards and assist with caregiver qualifications and documentation.
  • Provider Advocacy: Build collaborative partnerships with local community organizations to promote the benefits of the Givers program.
  • Representative Role: Represent the organization at occasional provider advocacy and partnership meetings.
Requirements
  • Experience: Minimum two (2) years of experience in home health Medicaid waiver programs or similar fields, preferably working with elders or people with disabilities.
  • Valid Driver's License: A valid driver's license and a car to travel to the homes of member families.
  • Education: Bachelor's degree in social work or an equivalent combination of education and experience in related fields.
  • Skills: Strong communication, relationship-building, assessment, and problem-solving skills.
  • Technical Skills: Proficiency with case management systems and mobile applications.
  • Independence and Communication: Ability to work independently and maintain effective communication channels with all stakeholders.
  • Community Knowledge: Knowledge of community resources related to medical, housing, financial, and social issues for clients' care.
  • Compassion and Professionalism: Compassionate understanding and ability to handle stressful situations with empathy and professionalism.
  • Driver's License and Insurance: Valid driver's license and automobile insurance with the ability to travel within assigned geographic areas.
  • Organizational Skills: Strong organizational skills and meticulous attention to detail for compliance and regulatory standards.
  • Software Proficiency: Proficient in Zendesk (or other CRMs), Google Drive, and other software tools.
