Key Responsibilities- Provide nursing care for patients, members, and families to restore and promote health and to prevent illness and injury.
- Utilize the nursing process to evaluate patient needs and provide nursing care to promote health and wellness.
- Collaborate with the healthcare team to deliver high-quality, safe care and service.
- Facilitate transitions and coordination of care across the continuum.
- Accurately complete screenings, addressing barriers and care gaps, and schedule appointments within the appropriate discipline.
- Provide education based on national guidelines, specific to screening test results, and refer to appropriate resources.
- Set up, perform, and/or assist with procedures, within scope of practice and skill.
- Monitor patient response during and post-procedure.
- Assess and triage patients, using critical thinking skills.
- Review client consent and other forms for completeness and legibility.
- Accurately document patient assessment, data, actions, interventions, and patient response.
- Assist with event setting and closing, ensuring equipment and forms are stored and tracked appropriately.
- Use critical thinking and clinical assessment skills to handle emergency situations.
- Demonstrate customer service skills, including diplomacy, patience, and problem-solving.
- Adjust to fluctuating work flows and changing assignments.
- Act as a resource to the team, resolving problems with dignity and respect.
- Provide clinical services in various settings, including clinics, employer group sites, and mobile health vehicles.
- Conduct outgoing telephone calls to patients, providing preventative outreach and follow-up care.
- Complete other department-specific procedures and nursing care, within scope of practice and skill.
- Assist with event logistics and coordination, including inventory, supplies, and event tracking.
- Participate in the development and maintenance of data collections and report requirements.
- Provide recommendations and implement process improvements.
- Attend and participate in departmental meetings, as required.
- Complete required staff development and other trainings, as requested.
- Participate in fostering a safe workplace, including employee job safety analysis and safety awareness.
- Lead or participate in quality improvement activities and initiatives.
- Serve as a role model for quality and service, and as a resource, coach, and mentor for other team members.
- Minimum two (2) years of current clinical experience.
- Associates Degree or two (2) years of experience in a directly related field.
- High School Diploma or General Education Development (GED) required.
- Registered Professional Nurse License (Georgia).
- Basic Life Support required at hire.
- Minimum one (1) year of experience in mobile healthcare or community health.
- A valid CDL Class B driver's license preferred.
- Bachelors Degree or four (4) years of experience in a directly related field.
