Job Title: Legacy Health Case Manager

JES Tech is seeking a skilled Legacy Health Case Manager to join our team. This role is responsible for facilitating and coordinating clinical case management services to prevent unnecessary disability related to injury, illness, and hazardous exposures at the Hanford Site.

Key Responsibilities:
  • Collaborate with clinic leadership to develop and implement methods to achieve established goals and objectives.
  • Develop and periodically review processes, policies, and procedures to ensure current, evidence-based, occupational health best practices and standard of care.
  • Maintain records for Legacy Health Program and related case management activities.
  • Prepare reports including information related to case management, related medical service activities, correspondence, and communication.
  • Work closely with and provide reciprocal backup support to the Beryllium Case Manager.
Operations / Communications:
  • Serve as the clinical point of contact for the Legacy Health Case Management Program.
  • Interface with stakeholders to maintain multi-directional communication that supports efficient, high quality case management.
  • Provide counseling and education on the Legacy Health Program according to 10 CFR 851.
  • Coordinate referrals for screening, evaluations, and work capacity/suitability evaluations, and other consultations related to case management issues.
  • Communicate with patients and serve as point of contact for follow-up after a new injury or exposure event, including test results, work restrictions, and return to work.
  • Help patients understand and navigate fitness for duty assessments, work capacity/suitability evaluations, and return to work services.
  • Work with patients, providers, and employers to clarify medical restriction recommendations and identify potential accommodations, as needed.
  • Consult with medical, preventive, and behavioral health staff to coordinate a treatment plan that supports timely and safe return to work.
  • Coordinates clinical services and care plans with patients, providers, and employers.
  • Other related duties as assigned.
Patient Care:
  • Establish a compassionate, culturally sensitive, patient-centered care environment.
  • Educate patients and promote health, wellness, and lifestyle choices to improve well-being.
  • Participate as a member of the Exposure Response Team (along with the SOMD and other members of the Legacy Health Management Team), facilitating communication between patients, providers, and employer representatives related to emergency or exposure response.
  • Identify signs and symptoms of injury, illness, and hazardous exposure and provide care according to nursing protocols, policies, and procedures.
  • Provide safe, high quality nursing care in accordance with internal policies and procedures, local, state, and federal regulations, contract requirements, AAAHC standards, and standard of care.
  • Protect patients, co-workers, and self by adhering to standard precautions for infection prevention, OSHA, DOE, and site-specific safety and health policies and procedures, and AAAHC accreditation standards.
Requirements:
  • Bachelor of Science in Nursing (BSN) is required.
  • Valid, unrestricted Washington State registered nurse (RN) license is required.
  • Minimum of three years' experience in occupational health nursing with documented case management experience.
  • BLS and ACLS certification are required.
  • Certified Occupational Health Nurse (COHN) is desired.
  • Certified Case Manager (CCM) is preferred.
