About the Role

We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team at Thrive Skilled Pediatric Care LLC. As a Pediatric Home Health Nurse, you will play a vital role in providing high-quality clinical home care to medically fragile children, enabling them to grow and flourish to their full potential.

Responsibilities
  • Provide skilled pediatric home care to patients in their homes, focusing on their unique needs and requirements.
  • Develop and implement individualized care plans in collaboration with the patient's healthcare team.
  • Conduct thorough assessments, identify areas of concern, and make recommendations for improvement.
  • Administer medications, perform treatments, and provide education to patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
Requirements
  • RN/LPN with a valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
