We are seeking a skilled and compassionate Registered Nurse to join our Home Health team as a Case Manager. As a key member of our team, you will be responsible for coordinating home care services, establishing and maintaining patient plans of care, and ensuring the quality, customer service, and financial management of your assigned caseload.
Responsibilities- Conduct admissions, recerts, ROC, and discharges for the assigned case load.
- Conduct routine nursing visits as needed.
- Assess patient needs.
- Coordinate patient care between the patient, caregiver, physician, and other appropriate disciplines.
- Responsible for the completion of all home health documentation.
- Graduate of an accredited school of nursing required.
- Associate's or Bachelor's degree in nursing.
- Current valid North Carolina nursing license required.
- Basic Life Support (BLS) through American Red Cross or American Heart Association required.
- Driver's License Class C.
- Minimum 1 year of home health or related experience required.
