Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team in a Weekend shift position. As a Home Health Registered Nurse, you will be responsible for coordinating home care services, establishing and maintaining patient plans of care, and providing high-quality patient care.

Responsibilities
  • Conduct admissions, recerts, ROC, and discharges for assigned case load.
  • Conduct routine nursing visits as needed.
  • Assess patient needs and coordinate patient care between patient, caregiver, physician, and other appropriate disciplines.
  • Responsible for completion of all home health documentation.
Requirements
  • Graduate of an accredited school of nursing required.
  • Associate's or Bachelor's degree in nursing.
  • Current valid North Carolina nursing license required.
  • Basic Life Support (BLS) through American Red Cross or American Heart Association required.
  • Driver's License Class C.
  • Minimum 1 year of home health or related experience required.
