We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Novant Health. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and coordination in the comfort of our patients' homes.
Responsibilities:
- Conduct routine nursing visits to assess patient needs and develop personalized care plans.
- Coordinate patient care with physicians, caregivers, and other healthcare professionals to ensure seamless transitions and optimal outcomes.
- Manage and document patient records, including progress notes and treatment plans.
- Collaborate with interdisciplinary teams to develop and implement effective care strategies.
- Provide education and support to patients and their families to promote health literacy and self-management.
Requirements:
- Graduate of an accredited school of nursing with a current valid North Carolina nursing license.
- Associate's or Bachelor's degree in nursing.
- Minimum 1 year of home health or related experience.
- Driver's License Class C.
