Role Summary: Under the guidance of the Transportation Manager, this role is responsible for the safe and efficient transportation of individuals with physical limitations to their designated locations. The position necessitates regular communication with caregivers and community organizations.

Qualifications

Education: High School Diploma or equivalent

Experience: Must be at least 18 years old and possess a minimum of 1 year of experience working with elderly individuals requiring assistance.

Licensing Requirements: A valid North Carolina Driver's License with a clean driving record, free from major traffic violations.

Specialized Training: Ability to collaborate effectively within a team and operate vehicles designed for accessibility. Must adhere to the established competencies for the role as outlined by Kintegra Health and approved by CMS before independent operation and annually thereafter.

Health Requirements: Must be medically cleared for communicable diseases and have all vaccinations current prior to engaging in direct participant interaction.

Key Duties and Responsibilities:

  • Communicate effectively with participants, families, staff, and other relevant agencies.
  • Trained to operate various vehicles within the organization's transportation fleet, with the understanding that driving assignments may vary as directed by the Transportation Coordinator or their designee.
  • Transport individuals with mobility challenges using a specially equipped van, ensuring that all wheelchair lifts and securement devices are used correctly and that participants arrive at their appointments punctually.
  • Adhere to established schedules, making necessary adjustments to accommodate unforeseen changes, while determining the safest and most efficient routes for pick-up and delivery. Any schedule modifications must receive approval from the Transportation Manager or Scheduler.
  • Assist participants in boarding and disembarking from vehicles, employing proper safety techniques and maintaining a courteous demeanor. Demonstrate appropriate use, care, and storage of durable medical equipment at all times.
  • Provide door-to-door service, ensuring participants exit and enter their residences safely, particularly for those at risk of falls.
  • Respond appropriately to emergency situations.
  • Inspect and maintain the cleanliness and safety of the vehicle, reporting maintenance needs promptly. Complete daily maintenance checklists and submit them to the Transportation Coordinator or designee as required.
  • Maintain accurate documentation of fuel receipts as instructed by the Transportation Coordinator or designee.
  • Participate as a member of the Interdisciplinary Team when requested. Attend staff meetings and contribute to participant care planning as needed. Relay any changes in participant conditions to the appropriate team members.
  • Complete necessary documentation as required.
  • Maintain a safe driving record, verified annually through the retention of a valid North Carolina driver's license.
  • Meet all physical demands of the position, including standing, bending, reaching, pulling, pushing, squatting, and walking for extended periods.
  • Perform other duties as assigned.
