About the Role:

We are seeking a skilled Care Navigator to join our team at Oaks Integrated Care. In this role, you will work closely with our Access and Admissions department to facilitate linkage into our clinical mental health and substance use treatment programs.

As a key member of our team, you will be responsible for considering the various treatment needs of individuals and assisting them in navigating the treatment continuum. Our programs cater to adults, children, and families, providing a comprehensive range of services.

Responsibilities:

  • Collaborate with interdepartmental clinical teams and referral sources to ensure seamless care coordination.
  • Navigate the continuum of care with individuals seeking treatment, identifying the most suitable program options.
  • Assist in completing enrollment documentation and establishing future appointments to facilitate a smooth transition into care.
  • Consider the holistic needs of individuals served, linking them with all applicable services to provide a comprehensive support network.
  • Possess basic knowledge of mental health diagnostic criteria to accurately assess client needs.
  • Support the Access Coordinator in maintaining accurate data systems and ensuring adherence to standards set by the Agency and funding sources.
  • Proficiency in Microsoft applications and general computer operations is essential for successful performance in this role.

Qualifications:
