Nurse Care Coordinator

Job Summary
This is a full-time position that will work $85,000 - $110,000 per year, depending on experience. The Registered Nurse will be responsible for providing quality care to patients through a team effort, working with others to deliver personalized treatment plans. This includes executing policies and procedures to ensure patient safety, providing support for pain management, and assisting patients and families in identifying comfort measures.
Duties and Responsibilities
* Provide quality care to patients through a team effort
* Assure patient safety by executing appropriate policies and procedures
* Assist patient/family in identifying individualized comfort measures
* Follow up on internal resources to determine the outcome of interventions completed by other departments
* Mobilize resources in complex cases to maximize patients' control and participation over their own recovery
* Provide information and interpretation of the patient's condition and offer coping mechanisms to both patient and family
* Assess how much information a patient wants and needs while utilizing a vocabulary and approach that enables the patient to successfully process the care and course of treatment
* Formulate and document a discharge plan that maximizes the patient's ability to continue with meaningful life activities
* Integrate assessment and diagnostic information with intuition to foresee potential age-specific healthcare needs and anticipate patients' needs
* Ensure handoff communication and include a report in terms of the situations most likely to develop and the problems awaiting patients
* Identify proactively issues to be resolved related to patient education for medication management
* Provide discharge planning which includes instructions on discharge medications
* Use discretionary judgment to appropriately modify patient care regimens
* Facilitate appropriate response from other health care team members to provide quality and safe care
* Explore multiple aspects of care regimen with case manager and health care providers to promote appropriate resolution
* Display ability to see the entire unit and self-motivated to develop a plan for adjusting staffing patterns as needed
* Attend educational sessions when offered in areas that will improve one's ability to assess the needs on one's respective department
* Plan and provide unique and individualized comfort measures while utilizing intuitive and innovative approaches which are scientifically sound and evidenced-based practice
* Participate in unit-based and/or hospital-based committees
* Participate in performance improvement activities at the unit and/or hospital-wide level
* Participate actively in the preparation of Joint Commission readiness
* Participate in unit-level effort to achieve successful Patient Satisfaction scores and have an awareness of the HCAAPS measurements
* Perform any other related duties as required or assigned.
