About the Role

We are seeking a skilled Clinical Social Worker Level II to join our team at Mass General Brigham. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support.

Job Summary

The successful candidate will have the opportunity to work in a dynamic and challenging environment, where no two days are ever the same. You will be responsible for conducting comprehensive assessments, developing treatment plans, and collaborating with multidisciplinary teams to deliver high-quality patient care.

Key Responsibilities
  1. Conduct thorough psychosocial assessments of patients and families to identify social, emotional, and environmental issues.
  2. Develop biopsychosocial assessment, disposition, and treatment plans that address individual needs and goals.
  3. Collaborate with other professionals to evaluate patients' medical or physical condition and assess client needs as part of an interdisciplinary team.
  4. Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plans.
Requirements

  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Strong problem-solving and critical thinking skills.
  • Knowledge of internal and external resources and ability to refer clients appropriately.
