About the Role

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a vital role in providing end-of-life care to patients and their families.

Key Responsibilities
  • Manage and coordinate all aspects of patient care, including symptom management, pain relief, and emotional support.
  • Conduct comprehensive assessments of patients' physical, emotional, and psychological needs to develop and adjust personalized care plans.
  • Provide education and guidance to patients and their families about the dying process, care techniques, and available resources.
  • Maintain detailed records of patient care, treatment plans, and interactions, ensuring all documentation meets regulatory requirements and organizational standards for quality and compliance.
Requirements
  • Indiana RN license, or ability to obtain
