We are seeking a highly skilled and compassionate Senior Patient Care Manager RN to join our team at Heartland Hospice. As a key member of our clinical leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.
Key Responsibilities- Manage and oversee the clinical and administrative functions of assigned hospice sites
- Supervise and direct the work of team members, including RNs, LPNs, and CNAs
- Ensure timely completion of patient assessments, care plans, and other clinical documentation
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care as needed
- Respond to phone referrals and inquiries in a timely and professional manner
- Current RN licensure in the state of employment
- Minimum 3 years of experience in direct patient care, with at least 1 year in a leadership or management role
- Experience in hospice or home care preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
