Patient Care Manager Senior RN

We are seeking a highly skilled and compassionate Senior Patient Care Manager RN to join our team at Heartland Hospice. As a key member of our clinical leadership team, you will be responsible for the day-to-day operations of our hospice sites, ensuring the provision of high-quality care to our patients and their families.
Key Responsibilities- Manage and oversee the clinical and administrative functions of our hospice sites
- Supervise and direct the work of our nursing staff, ensuring they have the necessary resources and support to provide excellent patient care
- Develop and implement policies and procedures to ensure compliance with regulatory requirements and industry standards
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care as needed
- Current RN license in the state of employment
- Minimum 3 years of experience in direct patient care, with at least 1 year in a leadership or management role
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Strong clinical knowledge and skills, with the ability to assess patient needs and develop effective care plans
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
