Job Summary:The HIV Community Health Worker will provide medical case management, advocacy, and advisory assistance to HIV/AIDS patients. The successful candidate will offer support and linkage services, advocacy, education, counseling, and information to ensure access to effective healthcare and supportive services.
Key Responsibilities:- Support patients in engaging and remaining in HIV medical care.
- Coordinate medical treatments and provide follow-up support to ensure consistent engagement.
- Accompany non-English speaking HIV patients to medical or dental appointments outside the health center.
- Provide treatment adherence counseling in conjunction with the HIV Adherence nurse.
- Conduct brief assessments of patients twice yearly.
- Interpret for HIV clients who visit the health center for medical purposes.
- Inform patients of community resources, such as SNAP, and assist patients in accessing these.
- Advocate for patients and their families, taking into account unique cultural backgrounds.
- Participate in outreach and education activities.
- Work with the Program Supervisor and HIV Services team to provide culturally congruent services.
- Document all activities as required by the program.
- Bachelor's degree in Social Work or related field preferred or certification in Community Health within one year of hire.
- Multilingual in English, Spanish, Cape Verdean Creole, Portuguese essential.
A busy community health center located in a downtown business district. Occasional long hours may be required. Some travel required to state and national conferences.
Physical Requirements:Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds. Mostly sedentary and interoffice walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.
Brockton Neighborhood Health Center is committed to providing a safe and healthy environment for patients and staff.
