Key Responsibilities:
As a Registered Nurse, you will:

  • Conduct initial assessments of patients and their families to identify home care needs.
  • Perform comprehensive physical assessments and document medical histories.
  • Continuously evaluate patient nursing requirements.
  • Develop and adjust care plans based on patient status and needs.
  • Utilize health assessment data to establish nursing diagnoses.
  • Create care plans that set goals and incorporate therapeutic and preventive nursing actions, involving patients and families in the planning process.
  • Administer prescribed medications and treatments, ensuring compliance with the State Nurse Practice Act.
  • Provide counseling to patients and families to address nursing and related needs.
  • Deliver health care instructions tailored to patient assessments and care plans.
  • Identify discharge planning needs and implement them prior to patient discharge.
  • Act as a Case Manager when assigned, coordinating care for your caseload.

Qualifications:
To be considered for this role, you should possess:

  • A degree from an accredited nursing program and a minimum of one year of clinical experience.
  • Current California RN licensure and CPR certification.
  • Preferred experience in home health care.

Compensation:
Pay is structured on a per-visit basis, ranging from $100 to $170 per visit.
