Position Overview:
At Alameda Care Hospice (ACH), we prioritize delivering exceptional care to every patient, treating them with the empathy and compassion we would extend to our own family members. As a rapidly expanding agency in the San Francisco Bay Area, we provide essential patient care across multiple counties.

Key Responsibilities:
  • Leverage your clinical expertise to develop, implement, and manage comprehensive care plans for patients in their homes.
  • Conduct thorough initial and ongoing assessments of patients and their families to identify hospice needs.
  • Deliver professional nursing care by applying all aspects of the nursing process.
  • Draft and initiate individualized care plans.
  • Regularly reassess the needs of patients and their families/caregivers.
  • Participate in modifying care plans as required.
  • Utilize health assessment data to formulate nursing diagnoses.
  • Create care plans that set goals based on nursing diagnoses, incorporating palliative care strategies while involving patients and families in the planning process.
  • Initiate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.
  • Provide health education to patients based on assessments and care plans.
  • Assist patients with daily living activities, promoting self-sufficiency and comfort.
  • Act as Case Manager as assigned, coordinating care for your caseload.
  • Maintain accurate clinical documentation regarding patient conditions and care provided.
  • Communicate effectively with physicians about patient needs and changes in conditions.
  • Educate patients and families on self-care techniques and provide necessary instructions.
  • Collaborate with families and interdisciplinary team members to address emotional needs.

Qualifications:
  • Minimum of 1 year of experience as a Hospice Case Manager.
  • Valid and unrestricted RN license in California.
  • Current BLS/CPR certification.
  • Experience working within an interdisciplinary team is preferred.
  • Familiarity with case management principles is advantageous.
  • Self-motivated and organized with strong critical thinking skills.
  • Demonstrated leadership abilities.

Job Types:
Full-time, Part-time

Compensation:
$125,000 - $160,000 annually

Experience Required:
  • 1 year of hospice experience (required)
  • 1 year of case management experience (preferred)

License/Certification:
  • RN License (required)

Travel Requirements:
  • 100% travel is required.
