The Registered Nurse Case Manager (RN CM) is responsible for planning, organizing, and directing hospice services, with a strong background in nursing and community health education. The RN CM utilizes community resources to develop and oversee services tailored to the needs of individuals and families in their homes and communities. This role upholds and promotes the mission, values, and vision of VNA Health.
Key Responsibilities:
Patient Care Management
- Deliver comprehensive case management for hospice patients
- Conduct initial assessments of patients and families to identify hospice care requirements
- Perform thorough physical assessments and document medical histories
- Facilitate admissions to the appropriate level of care
- Evaluate patient status by:
- Developing and initiating a personalized plan of care
- Regularly reassessing patient and family needs
- Adjusting the plan of care as necessary
- Implement the care plan and modify it as patient needs evolve
- Utilize health assessment data to establish nursing diagnoses
- Create a care pathway that sets goals based on nursing diagnoses and incorporates palliative nursing interventions, involving the patient and family in the planning process
- Initiate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed
- Provide counseling to patients and families to address nursing and related needs
- Offer health care instructions based on assessments and care plans
- Assist patients with daily living activities and promote self-sufficiency and comfort care
- Maintain knowledge of regulations governing hospice service delivery
- Manage financial resources and eligibility under various payor sources, including Medicare and Medi-Cal
- Maintain accurate clinical documentation regarding patient conditions
- Record changes in pain/symptom management as appropriate
- Communicate with physicians about patient needs and report any changes in conditions; obtain necessary orders
- Coordinate care plans with community health professionals
- Educate patients and families on self-care techniques
- Provide medication, dietary, and other instructions as directed by physicians, and leverage opportunities for health counseling
- Collaborate with other members of the hospice interdisciplinary team
- Ensure a safe environment for patients
- Assist families and caregivers in maintaining continuity of care
- Work alongside families and interdisciplinary team members to address emotional needs
- Meet daily productivity standards set by the organization
- Participate in on-call and weekend duties as required
- Engage in Quality Management initiatives
- Ensure availability of necessary equipment and services
- Supervise and evaluate hospice aide care every fourteen days
- Participate in educational programs to enhance nursing skills and knowledge
- Adhere to daily productivity standards established by the organization
Qualifications:
- Current and active California RN license
- Bachelor's degree preferred
- Minimum one year of acute care experience required
- One year of recent hospice experience preferred
- Current CPR certification
- Valid driver's license with insured vehicle as per state and/or organizational requirements
- Management experience is not required; responsible for supervising hospice home health aides/LVNs
- Strong observation, verbal and written communication skills, problem-solving abilities, and basic math skills; nursing skills as per competency checklist
- Ability to respond independently to life-threatening situations
- Capacity to reason, deduce, and execute complex multi-step processes
- Bilingual in Spanish preferred, but not mandatory
- Ability to lift up to 25 lbs
- Ability to push/pull up to 50 lbs
- Ability to stoop, bend, and squat
- Ability to walk up one flight of stairs
- Ability to walk one mile
- Ability to sit for a minimum of one hour
- Visual and auditory acuity
