Position Overview:

Key Responsibilities:
  • Utilize your advanced clinical expertise to design, implement, and manage comprehensive care plans for patients in their residences.
  • Conduct thorough initial and ongoing assessments of patients and their families to identify hospice requirements, including complete physical evaluations and medical histories.
  • Deliver professional nursing care by applying all components of the nursing process.
  • Draft and initiate individualized care plans.
  • Regularly reassess the needs of patients and their families/caregivers.
  • Participate in modifying care plans as necessary.
  • Employ health assessment data to establish nursing diagnoses.
  • Formulate care plans that set achievable goals based on nursing diagnoses, involving patients and families in the planning process.
  • Initiate preventive and rehabilitative nursing actions. Administer medications and treatments as per physician directives.
  • Provide health education to patients based on assessments and care plans.
  • Assist patients with daily living activities, promoting self-sufficiency and optimal comfort care.
  • Act as Case Manager when designated by the Clinical Supervisor, coordinating care for assigned patients.
  • Maintain accurate clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.
  • Communicate effectively with physicians regarding patient needs and changes in conditions; obtain necessary orders.
  • Educate patients and families/caregivers on self-care techniques, medication, and dietary instructions as prescribed by physicians, while recognizing opportunities for health counseling.
  • Collaborate with families/caregivers and interdisciplinary team members to address the emotional needs of patients and their families.
