The Hospice Care Registered Nurse is responsible for delivering compassionate care and is adept at evaluating pain and other intricate symptoms during end-of-life care. This nursing professional collaborates with the interdisciplinary hospice team to strategize and implement services tailored to the needs of patients and their families in an inpatient environment. The Hospice RN embodies the Core Values and is dedicated to maintaining the Standards of Behavior upheld by VNA Health.
Key Responsibilities:
- Deliver comprehensive nursing care to patients at Serenity House
- Administer nursing services, treatments, and preventive measures in accordance with inpatient facility protocols
- Engage in interdisciplinary team meetings
- Contribute to the development, execution, and modification of the interdisciplinary care plan for each patient and family, involving them in the goal-setting process
- Evaluate patient responses to treatments and medications, and communicate findings to the attending physician and relevant team members
- Coordinate care with other team members and community resources as necessary
- Educate and support patients and families regarding pain management and the psychosocial aspects of care for those facing terminal illness
- Ensure patient safety is maintained at all times
- Instruct patients and families on self-care techniques as appropriate, providing guidance on medication, diet, and other physician-ordered instructions while identifying opportunities for health counseling
- Assist in ensuring continuity of care among patients, families, and team members
- Supervise LVNs, Home Health Aides, and Volunteers
- Facilitate arrangements for necessary equipment and services
- Participate in clinical education, in-service training, and nursing staff meetings
- Engage in Quality Management and Utilization Review Activities
- Maintain accurate and timely records and reports
- Apply knowledge of state, federal, and accreditation regulations governing hospice service delivery
- Document and submit precise clinical notes regarding patient conditions, including pain and symptom management changes
- Active California RN license
- Bachelor's degree preferred
- Minimum of one year of acute care experience required; experience in home health and/or hospice preferred
- Current CPR certification
- Experience with IV and Central Line procedures desired
- Strong observational, verbal, and written communication skills, along with problem-solving abilities
- Basic computer skills (email) required
- Ability to communicate effectively, demonstrate initiative, reliability, and sound judgment
- Capacity to work collaboratively within an interdisciplinary team as well as independently, especially in crisis situations
- Ability to manage emotional stress and respect diverse lifestyles without judgment
- Reliable transportation
- Ability to lift up to 25 lbs
- Ability to push/pull up to 50 lbs
- Ability to stoop, bend, and squat
