Patient Care Assistant

The primary objective of the Certified Nursing Assistant role is to deliver routine daily nursing care and services to assigned residents in alignment with their individualized care plans.
KEY RESPONSIBILITIES
Engages in duties and responsibilities within a nursing facility, which may encompass, but are not limited to, the following tasks:
- Committed to providing exceptional customer service.
- Maintains consistent attendance and punctuality.
Delivers personal care to residents in a manner that ensures their safety and comfort, adhering to facility policies and state/federal regulations.
Strives to honor resident preferences and choices in all aspects of care.- Identifies and rectifies safety hazards in patient care areas, understanding personal accountability for all facets of patient safety.
- Adheres to established policies, practices, and standards for nursing services.
- Encourages and supports the highest possible level of independence for residents.
- Communicates residents' status to the charge nurse at the beginning and end of each shift.
- Provides insights into residents' conditions and documents observations as required.
- Fosters a safe and clean environment.
- Follows procedures for safe lifting and/or transferring of residents in accordance with established guidelines.
- Ensures that equipment is clean and operational, reporting any malfunctions to the supervisor.
Performs incidental housekeeping or maintenance tasks as necessary to uphold a clean, hazard-free environment for residents, visitors, and staff.
Utilizes personal protective equipment as needed for personal care tasks.- Conducts regular rounds on the assigned unit to ensure the environment is tidy, odor-free, and orderly, making corrections as necessary. This includes resident rooms, hallways, public restrooms, shower rooms, dining areas, and other common spaces.
- Completes daily documentation as required, including restorative programming and computerized records.
- Implements restorative programs as directed.
- Is knowledgeable about the abuse prevention policy and understands the responsibility to enforce it.
- Performs other duties and tasks as assigned.

While executing the responsibilities of this role, the employee may occasionally be exposed to blood or other bodily fluids, fumes, airborne particles, and toxic or caustic chemicals.
The noise level in the work environment is typically moderate.The ability to communicate in English via phone, in writing, and verbally with various levels of staff, patient families, and external customers is essential.

Requirements:EDUCATION AND EXPERIENCE REQUIREMENTS
State certification as a Certified Nursing Assistant.
CPR certification preferred.
Ability to read, write, and speak the English language.
No disqualifying criminal offenses as defined by regulatory guidelines.
CONTINUING EDUCATION
Participates in in-service and educational programs.
Completes required in-services on time as assigned.
Completes 12 hours of in-service annually to maintain certification.
Keeps CPR certification current.
