Job Summary:

Mental Health Partners is seeking a dedicated and compassionate Health and Wellness Coach to join our team in providing comprehensive care and support to adults with severe and persistent mental illness. As a key member of our interdisciplinary team, you will play a vital role in promoting the recovery mission of MHP by assisting in identifying, developing, implementing, and facilitating programs and curricula that address the 9 domains of wellness.

Key Responsibilities:
  • Assist with case management needs as part of a larger interdisciplinary team
  • Identify and reduce barriers to community integration
  • Assist clients in managing risk as they take action to become self-responsible and independent
  • Meet with small groups of clients to assess and empower them to identify goals and take action steps to achieve their goals
  • Assist with the on-going development and coordination of workshops and classes, and additional trainings for clients, staff, and the community
  • Facilitate or co-facilitate classes, workshops, and trainings
  • Assist clients to integrate into physical health care treatment in the community
  • Provide information to clients about the importance of the integration of mind, body, and spirit in enhancing their recovery process and improving their overall health and quality of life
  • Monitor, report, and enter data as needed to track program effectiveness and outcomes
  • As a member of the Adult Behavioral Health Home, actively promote a culture of staff wellness
  • Promote, support, and demonstrate through both behavior and job performance and adhere to policies, procedures, on a day-to-day basis
  • Effectively interact and communicate with other MHP staff/clients/customers/partners/etc.
