Job Summary:

As a Case Manager with Mental Health Partners, you will provide supportive, community-based services to youth and young adults ages 15-29 who are experiencing their first episode of psychosis.

You will function on a wrap-around team to provide support around benefits, education, housing, and family collaboration.

The Case Manager is responsible for providing direct support and skills training in collaboration with individuals, their families, and community supports.

They will identify strengths and barriers within systems and utilize a strengths-based approach that focuses on early intervention strategies.

Requirements:

Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred or one-year experience in the behavioral health field for non-related Bachelor's degree.

Experience working with adults and/or children in an outpatient setting.

Must have current CO Driver's License and a safe driving record.

Your work location may fluctuate between the office and out in the community based on client need situations, 5 days a week.
