Job Summary

Rural Health Services Inc is seeking a skilled Nursing Manager to join our team. As a key member of our clinical leadership team, you will be responsible for coordinating care, educating patients, and building trust between patients, family supports, and medical providers.

Key Responsibilities
  • Assist patients in accessing and utilizing healthcare resources
  • Improve health outcomes through coordinating care and educating patients
  • Consult with healthcare team members to coordinate services and enhance communication and continuity of care
  • Manage, provide, and evaluate nursing care in the assigned area/team
  • Review provider schedules and individual charts to coordinate care and ensure daily team huddle occurs
  • Handle non-appointment related calls from patients and resolve issues or route to the appropriate party
  • Provide direct supervision to assigned clinical support staff and medical assistants
  • Maintain an appropriate inventory of pharmaceutical supplies, samples, and clinical supplies
  • Ensure provision of intake assessments and follow up with patients when barriers are identified
  • Utilize standing orders to ensure appropriate provision of laboratory testing, immunizations, and referrals
  • Support patients and providers in the medication refill process and facilitate patient medication management
  • Ensure all patients are tracked and data entered into systems for follow up and reporting
  • Coordinate with providers to ensure case management services are provided to patients with multiple co-morbidities
  • Work with the team to develop, implement, and carry out programs in chronic disease management
  • Identify and utilize cultural and community resources and monitor lifestyle factors affecting health
  • Provide individual and family educational interventions and support patient self-management of disease and behavior modification interventions
  • Participate in quality improvement activities and contribute to the development and maintenance of a library of educational resources
Requirements
  • Successful completion of an accredited School of Nursing and South Carolina licensure as a Registered Nurse
  • Experience with clinical teams, electronic health records systems, and working in clinical out-patient settings
  • Experience working with diverse population groups and ability to interact effectively with patients and staff
