The Role

The Practical Nursing Specialist plays a vital role within the Health & Wellness Team, delivering exceptional care to our residents under the guidance of registered nurses and healthcare professionals.

Key Responsibilities

ESSENTIAL FUNCTIONS:
  • Assisting residents with daily personal care, including bathing, grooming, and dressing.
  • Administering medications and supporting residents in their medication routines.
  • Repositioning residents in bed and accompanying them throughout the community.
  • Providing toileting assistance and managing incontinence care.
  • Monitoring vital signs, including blood pressure, heart rate, and pulse.
  • Observing and reporting changes in residents' health, ensuring timely escalation of any concerns.
  • Documenting daily reports for the Nurse or designated charge person at the end of each shift.
  • Offering emotional support and comfort to residents and their families.
  • Identifying abnormal changes in residents' health and reporting them to a supervisor.
  • Conducting nursing assessments, including vital signs monitoring and health evaluations, to promptly address resident needs.
  • Providing wound care and other nursing interventions as directed by the registered nurse or healthcare provider, adhering to state regulations and infection control protocols.
  • Responsibilities encompass customer service, safety, resident care, and effective communication.
Qualifications

POSITION REQUIREMENTS:
  • Must be at least 18 years of age.
  • Active Licensed Practical Nurse with an unrestricted license in the state.
  • Successful completion of State and Company criminal background/drug screenings.
  • High school diploma or equivalent.
  • Current CPR certification.
  • Compliance with CEU requirements.
  • Proficient in reading, writing, speaking, and understanding the English language.
  • Able to make independent decisions when necessary.
  • Ability to interpret and implement written or oral policies, procedures, and instructions.
  • Competence in assessment and resident care skills, including medication management principles.
  • Strong written and verbal communication skills.
