We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Patient Care Specialist. In this role, you will be responsible for providing high-quality patient care and support to individuals with intellectual, developmental, or cognitive disabilities.
Key Responsibilities- Provide 1:1 care to clients, following their individualized care plans and promoting their dignity and independence.
- Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
- Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
- Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
- Accurately document nursing actions and communicate with family members, case managers, and physicians.
- Order supplies and medications as needed to ensure adequate stock levels.
- Monitor incidents involving clients and ensure incident reports are completed timely and accurately.
- Ensure all medical equipment is maintained and report any faulty equipment to the Durable Medical Equipment team.
- Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR certification.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In-depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.
