Position Overview:

The Clinical Nurse Care Coordinator is responsible for overseeing the management, coordination, and facilitation of care activities for a designated patient demographic. This role is essential in achieving goals related to length of stay and resource utilization while ensuring the quality of service is maintained or enhanced.

Key Responsibilities:

  • Act as a leader within a multidisciplinary team to promote collaborative practice, coordinated care, and continuity of care.
  • Ensure consistent clinical outcomes and efficient management through quality improvement initiatives.
  • Implement the utilization review program in accordance with established management plans and assist in the collection of quality improvement data.
  • Provide guidance and leadership to staff, management, and physicians regarding compliance with Medicare/Medicaid regulations and third-party payer contracts.
  • Offer education and consultation to enhance clinical documentation and coding accuracy.
  • Engage in clinical performance improvement by identifying opportunities to enhance efficiencies and reduce costs.
  • Participate actively in governance, peer review, and educational support processes.

Qualifications:

  • Education or experience in Case Management and/or Utilization Review/Utilization Management.
  • Current Washington Registered Nurse License upon hire.
  • A minimum of three years of experience in the relevant field.
  • Preferred experience in clinical nursing, case management, or utilization management.

Preferred Skills:

  • Bachelor's Degree in Nursing.
  • Proficiency in computer technology and Microsoft Office applications.
