About the Role

We are seeking a skilled Clinical Coordinator to join our team at DaVita, a leading provider of dialysis services. As a Clinical Coordinator, you will play a critical role in delivering high-quality patient care and services in our outpatient dialysis center.

Key Responsibilities
  • Coordinate patient care and services, including trend lab values, vital signs, and outcome management.
  • Ensure the highest level of care is delivered to patients by ensuring their safety, comfort, and well-being.
  • Train and support clinical teams to deliver exceptional patient care.
  • Work collaboratively with interdisciplinary teams to achieve clinical goals and quality improvement initiatives.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
