Job Title: Registered Nurse

Job Summary:

Brown University Health is seeking a highly skilled Registered Nurse to join our team as a Registered Psychiatric Nurse (RPN) on the Healthy Transitions Team (HT). The RPN will provide screening assessment and psychiatric nursing services to youth and young adults struggling with severe mental illness and co-occurring substance use.

Responsibilities:
  • Provide psychiatric nursing interventions, referral, and care coordination services for enrolled youth and their families.
  • Conduct screenings, biopsychosocial assessments, and high-level care coordination to ensure follow-up and connection with referrals made.
  • Engage in trainings on Coordinated Specialty Care model, evidence-based wrap-around models, motivational interviewing, cognitive behavioral therapy, and practices, and supported employment.
  • Participate actively in care coordination with community and health providers and engage in multi-disciplinary review meetings to develop integrated care plans for enrolled youth and families.
  • Complete screenings utilizing standardized tools for substance use and mental health, including SBIRT, trauma screening, and tools for depression, anxiety, suicidal ideation, and social determinants of health.
  • Engage in screening for general physical health, including body mass index, sleep issues, asthma, oral health, immunizations, wellness visits, tobacco use, and exposure to communicable diseases.
  • Facilitate referrals as appropriate and conduct follow-up to ensure connection and follow-through.
  • Engage and maintain collaborative, positive relationships with a network of specialized providers to address the needs of the population, including nutrition education, physical activities, smoking cessation, health literacy, housing, social, and rehabilitative programs, and job training and placement.
  • Assist the team in timely completion of GPRA outcome tool.
  • Maintain standards for quality care, productivity, and completion of clinical documentation.
  • Assist the program manager in the completion of monthly progress reports and health reports to the Department of Behavioral Health, Developmental Disabilities, and Hospitals.
Requirements:
  • Licensure as a Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
  • Associate or bachelor's degree in Nursing.
  • Minimum of two years of experience working as a psychiatric nurse in a community-based behavioral health setting.
  • Valid and current Rhode Island Registered Nurse License.
  • Valid driver's license and use of a car during normal working hours or other means of transportation to complete major functions of the job.
  • Automobile liability insurance coverage for the vehicle being used in accordance with Center requirements.
  • CPR training within 90 working days of employment and maintained annually.
Preferred Qualifications:
  • Understanding of mental health and substance use issues.
  • Understanding of family systems model of care.
  • Knowledge of community resources.
  • Familiarity with screening/assessment tools and evidence-based practices.
  • Oral and written communication skills at a proficient level, ensuring effective and professional communication.
  • Ability to provide crisis intervention, brief counseling, or case management when determined necessary.
  • Ability to provide psychiatric/medical consultation to HT staff and/or community providers as appropriate to the position.
  • Diplomacy, strong collaboration, and care coordination skills.
  • Report writing abilities, including the formulation of clinical assessments and writing of monthly progress reports for grant purposes.
  • Ability to track and aggregate data for outcome measurement and reporting.
  • Ability to understand, embrace, and reinforce agency ethics and codes of conduct in all contacts with clients, coworkers, and the community.
