Psychiatric Nurse Specialist

Job Summary:
This is a part-time position as a Registered Psychiatric Nurse (RPN) on the Healthy Transitions Team (HT). The team provides outreach, comprehensive evidence-based treatment, referral, and support services for youth and young adults struggling with severe mental illness and co-occurring substance use.
Key Responsibilities:
- Maintain knowledge of the Healthy Transitions Coordinated Specialty Care evidence-based treatment model.
- Contribute to team planning and case review, providing psychiatric nursing interventions, referrals, and care coordination services.
- Provide education to clients and families, supporting them in achieving their goals.
- Attend agency and Healthy Transitions staff meetings, as well as weekly supervision.
- Maintain accurate and timely documentation in medical records.
- Enhance professional expertise through educational activities and trainings.
- Participate in care coordination with community and health providers, engaging in multi-disciplinary review meetings to develop integrated care plans.
- Engage in cross-training of ICT staff and FQHC Clinician on Serious Emotional Disturbance, Serious mental illness, substance use disorders, chronic disease, and health conditions.
- Complete screenings using standardized tools for Substance use and mental health, including SBIRT, trauma screening, and tools for depression, anxiety, suicidal ideation, and social determinants of health.
- Engage in screening for general physical health, facilitating referrals as appropriate and conducting follow-up to ensure connection and follow-through.
- Engage and maintain collaborative relationships with specialized providers to address population needs, including nutrition education, physical activities, smoking cessation, health literacy, housing, social, and rehabilitative programs, and job training and placement.
- Assist the team in timely completion of GPRA outcome tool.
- Maintain standards for quality care, productivity, and completion of clinical documentation.
- Assist the program manager in completing monthly progress reports and health reports to the Department of Behavioral Health, Developmental Disabilities, and Hospitals, tracking and submitting identified metrics.
Requirements:
- Licensure as a Registered Nurse in the State of Rhode Island or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
- Associate or bachelor's degree in Nursing.
- Minimum of two years of experience working as a psychiatric nurse in a community-based behavioral health setting.
- Valid and current Rhode Island Registered Nurse License.
- Valid driver's license and use of a car during normal working hours or other means of transportation to complete major functions of the job.
- Automobile liability insurance coverage for the vehicle being used in accordance with Center requirements.
- CPR training within 90 working days of employment and maintained annually.
Preferred Qualifications:
- Understanding of mental health and substance use issues.
- Understanding of family systems model of care.
- Knowledge of community resources.
- Familiarity with screening/assessment tools and evidence-based practices.
- Oral and written communication skills at a proficient level, ensuring effective and professional communication.
- Ability to provide crisis intervention, brief counseling, or case management when determined necessary.
- Ability to provide psychiatric/medical consultation to HT staff and/or community providers as appropriate to the position.
- Diplomacy, strong collaboration, and care coordination skills.
- Report writing abilities, including the formulation of clinical assessments and writing of monthly progress reports for grant purposes.
- Ability to track and aggregate data for outcome measurement and reporting.
- Ability to understand, embrace, and reinforce agency ethics and codes of conduct in all contacts with clients, coworkers, and the community.
