Job Summary:

Leoforce is seeking a skilled Registered Nurse to join our Home Care team. As a Registered Nurse, you will be responsible for providing high-quality patient care and coordination in a home setting.

Key Responsibilities:

  • Conduct admissions, recerts, ROC, and discharges for assigned case load
  • Conduct routine nursing visits as needed
  • Assess patient needs and develop individualized care plans
  • Coordinate patient care between patient, caregiver, physician, and other disciplines
  • Responsible for completion of all home health documentation

Requirements:

  • Graduate of an accredited school of nursing required
  • Associate's or Bachelor's degree in nursing
  • Current valid North Carolina nursing license required
  • Basic Life Support (BLS) through American Red Cross or American Heart Association required
  • Driver's License Class C
  • Minimum 1 year of home health or related experience required
