About the Role:

We are seeking a Patient Advocate Specialist to join our team at Volunteers of America Massachusetts. As a Community Support Navigator, you will play a critical role in providing recovery support navigation and community support services to individuals with mental health, substance use, dual diagnosis, and co-occurring disorders.

Key Responsibilities:

  • Engage with clients to identify needs, conduct initial assessments, and write service plans to support their goals and objectives.
  • Provide follow-up support to ensure clients access treatment services, appointments, and resources, including primary care, mental health treatment, addiction treatment, recovery support, employment, and housing supports.
  • Establish and maintain relationships with referral providers to ensure coordination and success of services.
  • Serve as part of an integrated services care team, coordinating care with other providers to enhance client experiences and support a wrap-around service model.
  • Maintain accurate records according to VOAMASS policies and procedures, completing documentation and authorizations promptly.

Requirements:

  • Bachelor's Degree in psychology, social work, healthcare administration, or related human service fields (desired).
  • Minimum two years' experience with social service organizations, case management coordination, and service connection.
  • Excellent communication, organization, and computer skills, valid driver's license, and reliable transportation required.
