About the Role:

Overview: We are seeking a skilled Healthcare Professional to join our team as a Healthcare Population Specialist. As part of our Itasca County Health and Human Services department, you will play a crucial role in improving the health outcomes of our community members.

Salary: $60,000 - $80,000 per year. Salary range is determined by experience and qualifications.

About the Job:

  • Provide indirect population interventions for IMCare members, including research and review of evidence-based guidelines, case management, and data analysis.
  • Collect and analyze specialized data to prepare accurate reports on quality improvement and compliance with DHS contracts and NCQA standards.
  • Manage the population health management program, identifying suitable members and providing ongoing assessment and intervention.
  • Collaborate with peers to address provider, pharmacy, and member requests in a timely manner.
