Mobilizing Navigator for Youth Care

GeorgiaGov has partnered with the Georgia Department of Behavioral and Developmental Disabilities to address the needs of youth in Region 3, aged 6-21, who are experiencing health crises and barriers to accessing appropriate care and support.

This partnership enables us to develop and pilot a formalized multi-agency care coordination protocol to remove barriers and expedite access to the right care at the right time for youth and families.

Key Responsibilities:
  • Collaborate with multi-agency teams to provide comprehensive care coordination services
  • Develop and implement individualized care plans to address the unique needs of each youth
  • Facilitate access to community resources and services to support youth and families

Requirements:

  • High School diploma/GED and three (3) years of job-related experience in a social services-related position
  • One (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position
