Job Summary

The Assessment and Referral Coordinator plays a critical role in providing assessment and admission services to address patients with behavioral health needs. This position is responsible for receiving referrals, establishing assessment services, and ensuring that patient information is obtained accurately and conveyed to the necessary entities.

Key Responsibilities
  • Receive and process referrals from various sources, including healthcare providers and community agencies.
  • Conduct thorough assessments of patients to determine their behavioral health needs and develop appropriate treatment plans.
  • Collaborate with internal and external stakeholders to establish assessment and admission services, ensuring that patients receive the necessary care and support.
  • Ensure that patient information is accurate, complete, and conveyed to the necessary entities, including healthcare providers, insurance companies, and other relevant parties.
Qualifications
  • Prior experience with crisis intervention and assessment and referral processes is highly desirable.
  • Knowledge of psychiatric diagnosis and treatment options is essential.
  • Must demonstrate commitment and adherence to STHS's Compliance Program and Code of Conduct through compliance with all policies and procedures, attendance at required training, and immediate reporting of suspected compliance issues to the Compliance Officer.
Education and Licensure
  • A Master's Degree in psychology, sociology, counseling, or a related field is required.
  • Texas State Licensure is required and must be maintained current thereafter (Licensed Professional Counselor, Licensed Professional Counselor Associate, Licensed Master Social Worker, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist).
