Job Summary:

The Rehabilitation Liaison plays a vital role in receiving patient referrals and arranging evaluations. This individual secures appropriate contact information, assesses patient rehabilitation potential, and determines suitability for hospitalization. They interview patients and families to gather information and assess special needs. This position requires adherence to admission criteria and presenting potential patients for approval at admissions meetings. Additionally, the Rehabilitation Liaison coordinates pertinent data with the Business and Admitting Office and represents the facility in contracts with other health professionals and the general public.

Responsibilities:

  • Receive patient referrals via phone or fax and arrange for evaluations
  • Secure appropriate contact information and assess patient rehabilitation potential
  • Determine suitability for hospitalization and interview patients and families to gather information
  • Adhere to admission criteria and present potential patients for approval at admissions meetings
  • Coordinate pertinent data with the Business and Admitting Office
  • Represent the facility in contracts with other health professionals and the general public

Qualifications:

  • Two years of clinical experience in medical or one year of clinical experience in a physical rehab setting preferred
  • Good organizational and liaison capabilities with well-developed written and verbal communication abilities
  • Demonstrated public relation abilities with a cooperative and assertive attitude
  • Working knowledge of Medicare guidelines for Rehabilitation
  • Commitment and adherence to STHS's Compliance Program and Code of Conduct

Education/Licensure:

  • Bachelor's or Associate's degree from an accredited university and functional scope licensure/certification to function as a PT, OT, ST, or RN
  • Current RN, Physical Therapy, Occupational Therapy, or Speech Therapy licensure in the State of Texas and maintain licensure and certifications
  • BLCS certification, according to unit specific needs
