Nurse Care Coordinator

Job Summary:
Regency Integrated Health Services is seeking a skilled and compassionate Registered Nurse to fill the role of RN Charge Nurse. This position is responsible for ensuring the provision of quality care that promotes the highest practicable physical, mental, and psychological well-being of each resident.
Key Responsibilities:
• Observes, assesses, documents, and reports resident condition/changes during rounds
• Assures implementation of care plans as directed
• Administers medications as prescribed
• Conducts rounds with physician and records visits in clinical records
• Performs narcotic reconciliation with incoming and outgoing shifts
• Identifies significant changes in the condition of residents and takes necessary action per facility policies and procedures
• Oversees and monitors aides
• Initiates and completes Incident and Accident Reports per facility policy
• Discharges residents per facility policy
• Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition
• Communicates with resident's point of contact when they ask for a status update or if there is a change in condition
Requirements:
• Has completed a RN program at an accredited vocational school, college, or university
• Must be an active Registered Nurse in the state of Texas in good standing
• Experience working in a skilled nursing facility is preferred
