Job Summary

We are seeking a dedicated and compassionate Registered Nurse to join our team as an Assessment and Referral Coordinator. In this role, you will play a critical part in establishing assessment and admission services to address patients with behavioral health needs.

Key Responsibilities
  • Receive referrals and coordinate assessment and admission services for patients with behavioral health needs.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
  • Develop and maintain relationships with community resources and healthcare providers.
  • Ensure compliance with all policies and procedures, as well as adherence to the Code of Conduct.
  • Provide exceptional patient care and support to individuals with complex behavioral health needs.
Requirements
  • RN - Registered Nurse currently licensed in the State of Texas, or part of a compact state license, or has a valid temporary RN license for the State of Texas while waiting to receive the permanent license.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
