Comunilife Inc. is seeking an experienced Case Manager for its new sanctuary program in the Bronx. The Case Manager will conduct intakes, initial skills assessments, and provide case management services for new immigrant and refugee clients. The program services families, so case managers will work with the whole family unit, providing support for access to education, child care, after school services, and other needed family support.

Key Responsibilities:

  • Screens potential clients for program enrollment and orient clients to the program and describes program objectives.
  • Conducts initial intake assessments and utilizes assessment tools to identify support service needs and makes immediate referrals.
  • Provides assistance to clients in the job placement process, including taking clients to interviews, providing job case management, and helping with application completion.
  • Tracks and monitors participant progress, oversees weekly follow-up via one-on-one sessions, and follows up with participants through mentoring and counseling.
  • Maintains confidential case records for all clients and maintains contacts with outside agencies to refer or obtain additional client services.
  • Conducts collaborative efforts with service providers to maximize service delivery and facilitates workshops addressing issues related to career development, skill building, and résumé drafting.

Requirements:

  • Bachelor's Degree in human services or related field with two years' case management work experience.
  • Associates Degree in human services or related field with four years' case management work experience.
  • High School Diploma with six years' case management work experience will be considered.
  • Bilingual Spanish strongly preferred.
  • Good writing skills required.
  • Some customer service and job readiness experience preferred.
  • Knowledge of at least one of the following service areas is required: homeless-related issues, refugee, and/or asylum population.
