The Healthcare Liaison Specialist plays a crucial role in facilitating the delivery of services for patients within a healthcare facility. This position involves managing the admission process and collaborating with the admissions and marketing teams to foster and sustain relationships with referral sources for designated facilities.

Qualifications

  • Valid state license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Current CPR certification is required; additional nursing specialty certifications are preferred.
  • Proven understanding of community resources.
  • A minimum of two years of experience in community nursing, home health care, or hospice services.
  • Preferred two years of experience in marketing and community outreach.
  • Knowledge of payer sources and case management processes.
  • Exceptional written and verbal communication skills, along with strong interpersonal abilities.
  • Willingness to travel extensively, with the ability to work evenings and weekends as necessary.
  • Capacity to collaborate effectively with an interdisciplinary team and healthcare providers.
  • Ability to build and maintain rapport with clients and referral sources.
  • Proficient in analyzing and interpreting professional literature, technical procedures, and regulatory guidelines.
  • Strong problem-solving skills, with the ability to collect data, establish facts, and draw valid conclusions.

Key Responsibilities

  • Gather patient information during the referral process.
  • Accept patients on behalf of the facility following clinical and financial evaluations.
  • Educate referral sources about services through written and verbal communication.
  • Collaborate with hospital discharge planners to ensure a seamless discharge process.
  • Assist hospital and assisted living discharge planners in coordinating discharges, including family communication and transportation arrangements.
  • Facilitate communication between nursing staff at the facility and hospital/assisted living facilities to ensure continuity of care.
  • Support and review Emergency Department admissions as they arise.
  • Conduct monthly meetings with hospitals to review case statistics and discuss process improvements.
  • Analyze reasons for patient readmissions with hospital staff monthly.
  • Provide training and education to community organizations as required.
  • Act as a liaison among referral sources, patients, and their families.
  • Prepare reports for leadership as requested.
  • Admit patients to facility services, ensuring accurate documentation and consent.
  • Adhere to established work schedules and complete assigned tasks efficiently.
  • Follow agency policies and regulatory guidelines during referral and intake processes.
  • Engage in quality improvement initiatives within the agency.
  • Assist in developing effective care plans prior to patient transfers.
  • Meet or exceed established productivity benchmarks.
  • Recommend alternative care options, including community resources when appropriate.
  • Evaluate patients for service admission.
  • Advocate for patients to ensure they receive entitled benefits.
  • Participate in the referral process for ancillary services.
  • Follow up on referrals that have not been admitted to care.
  • Collaborate on marketing action plans for assigned facilities.
  • Engage in growth meetings and relevant training sessions.
  • Update referral sources on the status of patients receiving services or recently discharged.
  • Stay informed about changes and trends in the healthcare market.
  • Maintain regular communication with referral sources to strengthen working relationships.
  • Contribute to the company’s growth and profitability through responsible resource management.
  • Accept additional assignments as needed.
  • Comply with established personnel policies.
