Key Responsibilities:

  • Conducting thorough assessments, MDS, and care plans as designated
  • Initiating care plans and facilitating related activities as required
  • Timely creation and distribution of monthly care plan calendars
  • Updating and maintaining all care plans and assessments as necessary
  • Monitoring and auditing clinical documentation to ensure accuracy and promptness
  • Notifying the Director of Nursing regarding ongoing issues with non-compliant documentation
  • Ensuring the confidentiality of Resident and Facility information at all times
Qualifications:
  • Current RN License
  • Prior experience with MDS 3.0 is essential
  • Experience in Long Term Care is mandatory
  • Availability to work up to 32 hours per week
  • Highly organized, professional, and self-motivated
  • Proficient computer skills
  • Exceptional communication abilities
  • A friendly demeanor and strong team player
Experience Required:
  • MDS 3.0: Minimum of 2 years
  • Long Term Care: Minimum of 2 years
Licensure:
  • RN (LPN considered with relevant experience)
