Job Description

Job Summary:

LifePath Inc. is seeking a highly skilled and compassionate Wellness Coordinator to join our team. As a key member of our staff, you will work closely with older adults to assess their needs, develop practical solutions, and access necessary resources to enhance their overall well-being.

Key Responsibilities:

  • Assess and Identify Needs: Conduct thorough assessments to identify the strengths, interests, and needs of older adults, and develop a plan to address these needs.
  • Coordinate Care: Work with older adults, caregivers, and community partners to create a care plan that meets their unique needs and goals.
  • Provide Coaching and Support: Offer coaching and assistance to older adults to help them develop practical solutions and access necessary resources.
  • Facilitate Access to Resources: Assist older adults in accessing community supports, services, and resources to enhance their well-being.
  • Communicate Effectively: Communicate regularly with older adults, caregivers, and community partners to ensure that their needs are being met and that they are receiving the support they need.

Requirements:

  • Education and Experience: Bachelor's degree in a human service-related field or equivalent experience working with older adults.
  • Skills and Qualities: Strong clinical skills, emotional intelligence, empathy, coaching skills, communication and interpersonal skills, critical thinking and problem-solving skills, time management and flexibility, documentation and computer proficiency, cultural sensitivity.

Benefits:

  • Competitive Salary: $21.60 - $24.00 per hour commensurate with experience.

Work Environment:

Work is split between indoor office work and traveling outdoors to meetings, trainings, and consumers' homes. Must be able to tolerate odors and pollutants, heat and cold of seasonal changes, and indoor temperatures.
